Settings -> Customizations -> Customize the System
Expand Entities -> Expand Account
Select Fields
Add New fields
Select Forms from the left navigation -> Select Information (or Account if using the new forms)
Drag and drop fields onto the form
Save and Close
Click back on Account in the left navigation
Click Publish from the top
